It doesn't work at all? It may sound like a stupid question but are you plugging it in before you load the game up? Also are you sure this controller works otherwise?
Welcome to RU. Sorry I don't have an answer for you but I'm sure someone else will be along eventually with something more technical.
Plug the controller in before you start the game.
Once the game starts it should auto detect the controller and set up the default buttons.
By default the d-pad and main fire buttons should be set on the controller. The start and coin buttons will still be on the keyboard though.
You can change the defaults by pressing Tab to open the menu and then changing the controls from there.
If it still is not working then make sure Windows is recognizing it.
Go to Control Panel
Start Devices and Printers
Find your controller
Right click and select properties
Make sure all of the buttons are working.
If the controller is not listed as a device or the buttons are not working then the problem is not in the game but somewhere else.

Try scrolling to the bottom of the page and change the style from default to beta and then try playing the game again. It will load it in beta mode. Maybe you will have better luck with that.
Type 'Game Controllers' in the box. No misspellings. It should appear in the search results.
Alternatively, go to Control Panel -> Classic View (if its not already set) -> Game Controllers
The box should pop up with your controller selected. Click "Properties". It should then give you a menu for testing the inputs. Do they respond when you push buttons, move sticks/pad?
